Transjugular Intrahepatic Portosystemic Shunt (TIPS)

55 years old Male patient, diagnosed case of Chronic Liver Disease and portal Hypertension admitted in Chirayu Medical College & Hospital on 2nd September 2019 with complaint of bleeding per rectum since 27th August 2019.

Summary of Events

19.08.2019 – Patient diagnosed Oesophageal & Rectal Varices on endoscopy & Colonoscopy respectively.

Banding of varices was done and Bleeding stopped.

28.08.2019 – Patient again came with the bleeding per rectum.

Endocryl Glue was injected to Rectal Varices. Bleeding was not stopped.

About 18 – 19 units of packed RBC transfused to the patient.

TIPS (Transjugular Intrahepatic Portosystemic Shunt) procedure was advised after discussion with Gastroenterologist & Interventional radiologist.

02.09.2019 – Patient admitted to Chirayu Medical College & Hospital, Bhopal for TIPS

( Transjugular Intrahepatic Portosystemic Shunt) procedure to reduce portal hypertention.

TIPS (Transjugular Intrahepatic Portosystemic Shunt) procedure was done in DSA lab by Dr. Pankaj Goyal (Interventional Radiologist) under general anaesthesia. Team member consists of Dr. Parth Rai (Anaesthesist) and Dr. Harsha Gupta (Critical Care Expert).

Procedure was uneventful.

Patient shifted to ICU under critical care expert Dr. Harsha Gupta.

03.09.2019 – Patient extubated from ventilator at 11.30 am. After extubation patient is alert, cooperative and vitals are stable.
